Free Athletic Physicals for Poland Athletes!

Thanks to Mercy Health and Kirstie Bournias free athletic physicals will be offered to all Poland student-athletes. This generous service is not typically offered at no cost, so please extend your gratitude to those making it possible.

Important Instructions:

  • Physical Form: Bring a printed copy of the physical form with all required parent/guardian signatures completed. Students without a signed form will not be seen.
  • Arrival: Arrive 5 minutes before your scheduled time and enter through the main Field House doors (facing the police station). Physicals will be held in the teacher’s lounge.
  • Sign-Up: Please register using your child’s name and Poland school email address. This information will be used to distribute baseline concussion testing.

Concussion Testing Requirements:

Baseline concussion testing is required for students in the following sports:

  • Football
  • Soccer
  • Volleyball
  • Cheerleading
  • Basketball
  • Wrestling
  • Baseball
  • Softball
  • Lacrosse.

It is mandatory for all 7th and 10th graders, as well as any student participating in one of the above sports for the first time. Tests will be emailed the week of July 20–24 from Athometest@impacttest.com. Follow the link in the email to watch the instructional video, then complete the test (approx. 30–40 minutes) on a computer or Chromebook. Tests are not compatible with cell phones.

Note: Concussion testing is not conducted during physical day.

  • Parents do not need to attend or remain during the physical.
  • Please allow approximately 30 minutes for your appointment.
